Vaccine Exemptions in Alaska

The state of Alaska provides a medical exemption form for all children attending public school, private school, or a child care facility. Documentation must be signed by an Alaska-licensed MD, DO, ANP, or PA, stating the child is medically exempt from receiving immunization(s).
A Religious Exemption (page 15) is also available for students attending public, private, or child care facilities and must be filed annually and notarization is required.
